My "business days" are Monday through Thursday

If you need a refill, contact me at least 2 business days in advance - do NOT assume I received a pharmacy request, as these automated faxes/messages are often lost

Let me know 24 hrs in advance if you need to reschedule - otherwise, there will be a $75 late cancellation fee. If you don't show up to an appointment, including if you contact me AFTER the start time of an appointment, you will owe the full out of pocket fee for the service ($150 for a follow-up, $300 for an intake)

I allow a 10 minute grace period to show up for appointments

You must keep a credit/debit card on file for copays and fees
